JFS Food Sort Registration

By completing this form I have agreed to assist Jewish Family Service (JFS) in a volunteer capacity by donating my services. I expect no pay, benefits, or compensation of any kind or in any way for my charitable assistance to JFS. 

I recognize and agree that some volunteer activities may be strenuous and involve certain risks. I expressly assume these risks, whether such risks are known or unknown to me at this time. I release and hold harmless JFS and its officers, directors, employees, agents, representatives, and sponsors, from and against any liability for injuries, property damage, and other losses or harm I may sustain in the course of my participation. 

I understand that any information I may obtain directly or indirectly from clients, staff, or other volunteers while I am volunteering at JFS is confidential and this confidentiality is protected by law. 

I agree and consent to be photographed and/or video recorded during the course of my volunteering with JFS, with the understanding that these materials may be featured in JFS marketing and promotional publications. I waive any claim for any compensation and/or damages for such use. I understand that I will have the opportunity to opt out of being photographed at each individual volunteer engagement I attend. 

I understand that my acceptance as a volunteer is based on the information I have provided to the agency, a personal interview, and that a routine background check will be performed. I certify that all statements made in my volunteer application are true and correct, and have been given voluntarily. I give JFS the right to investigate all information given, and release JFS and its agents from liability arising out of such investigation.
