Supported Living Program
The Supported Living Program (SLP) provides the levels of ongoing care and support that are needed so that clients in the Greater Seattle area can live and flourish in their own homes locally. This includes:
Intensive Case Management: Professional Case Managers work tirelessly to ensure that each individual client has the resources required to live in the community. Services are tailored to meet the unique needs of each participant including:
- Government & Community Resource Liaison
- Coordination of Health Care
- Payeeship
- Checking & Bill Paying Services
- Management of Petty Cash
- Menu & Dietary Planning
- Behavior Support
- Emergency Preparedness Planning
Instruction in Daily Living Skills: While encouraging clients to do as much for themselves as possible, SLP Instruction & Support Specialists provide coaching and assistance to participants who require help in maintaining their homes. This includes instruction with:
- Meal Preparation
- Grocery Shopping
- Medications
- Housekeeping
- Laundry & Clothing Care
- Transportation
- Personal Shopping
- Pet Care
- Plant Care
- Adaptive Technology
Assistance with Leisure Activities: Instruction and assistance is also available to clients who are interested in expanding their leisure activities including:
- Community Integration
- Friendly Visitors
- Religious Services
- Exercise & Physical Fitness
On-Call & Additional Services: Staff is available to respond to unanticipated client needs 24-hours a day, seven-days a week within the Greater Seattle area. Should a client and/or family desire the SAJD to respond to the individual’s needs in other emergent situations, a fixed number of additional staff hours may be pre-authorized. This is especially appealing for participants who do not have family in the immediate area.
Checking-In On Clients: SLP Case Managers are available to check-in on local residents if their guardians or family members reside outside of King County. Regular visits to monitor a client’s well-being are available.
